Guidelines
Cancellation/No Show Policy
The time and date you select is especially reserved for you. If for any reason you need to cancel or reschedule your appointment please give at least 24 hours notice. Please give 24 hours notice to avoid being charged the full session fee.
Client Intake Form
The client intake form must be signed and completed before any treatment takes place.
Conduct Policy
Clients are asked to avoid alcoholic beverages before the session. Consuming alcohol before treatments is contraindicated.
Ethics
Therapeutic massage is strictly non-sexual. Should the client exhibit any inappropriate behavior, the therapist will terminate the massage immediately and payment for appointed service will be rendered in full.
Feedback Policy
Feedback during and after the treatment is encouraged. In order to get the best possible benefit from treatment, open communication is essential.
Hygiene Policy
Both the therapist and client are required to follow proper hygiene etiquette. Due to health and safety regulations, the therapist reserves the right to refuse treatment due to improper hygiene. The therapist must follow universal precautions in order to protect the health and safety of both the client and the therapist
Payment Policy
Payment is due upon completion of the session unless treatment sessions have been previously purchased .e.g. gift certificate
Cash, Visa, MasterCard, Discover and American Express are accepted methods of payment.
A fee of $30 will be charged for returned checks.
Privacy Policy
Your information will be kept confidential. Information will only be shared with other health care providers for consults or coordination of care, or with the various agencies involved in payment reimbursement.
Tardiness Policy
If you are late for your scheduled appointment, the remaining time of your session will be honored. If the schedule permits some accommodation may be possible.
Waxing
Due to increased sensitivity while using Retin-A, Accutane, any other exfoliating medication we will not perform waxing on the area being treated. Burning or removal of the the skin may occur.
